About Psoriatic Arthritis

Psoriatic arthritis (PsA) is a chronic inflammatory arthritis occurring in 20–30% of people with psoriasis. It is characterized by peripheral joint inflammation, enthesitis (tendon/ligament insertion inflammation), dactylitis (sausage digits), axial involvement, and associated skin disease. Treatment has been transformed by biologics targeting TNF, IL-17A, IL-23, and the JAK/TYK2 pathways. Despite multiple approved agents, 40–50% of patients do not achieve adequate disease control.

What Types of Psoriatic Arthritis Clinical Trials Exist?

PsA clinical trials test next-generation IL-17 inhibitors, IL-23 inhibitors, TYK2 inhibitors, selective JAK inhibitors, and novel oral small molecules. Trials stratify patients by prior biologic exposure (naive vs. experienced), disease domain (predominantly peripheral, axial, enthesitis-predominant, or skin-predominant), and biomarker profiles. ACR20/50/70 response criteria and PASI scores for skin are standard endpoints.

Frequently Asked Questions

What clinical trials are available for psoriatic arthritis?
Recruiting PsA trials include IL-17 inhibitors, IL-23 inhibitors, TYK2 inhibitors (deucravacitinib), selective JAK inhibitors, and combination regimens. Many trials specifically enroll biologic-naive patients or patients who have failed one or more anti-TNF or IL-17 therapies.
Does my biologic history affect PsA trial eligibility?
Yes significantly. Trials are stratified by prior biologic exposure. Biologic-naive trials test new agents as first-line therapy. Biologic-experienced trials enroll patients with inadequate response, loss of response, or intolerance to one or more approved biologics. The number and mechanism of prior failed biologics matters.
Do I need both psoriasis and joint disease to qualify for PsA trials?
Most PsA trials require a confirmed PsA diagnosis (by CASPAR criteria), which includes active arthritis in the context of psoriatic disease. Active joint disease (swollen and tender joint counts) is typically required. Some trials also require active skin disease (BSA ≥3% or PASI ≥12), while others focus primarily on joint endpoints.
