Renal cell carcinoma (RCC) is the most common type of kidney cancer, representing 90% of cases. Clear cell RCC (ccRCC) is the most prevalent histology (70–80%) and is driven by VHL gene alterations. Treatment has been transformed by immune checkpoint inhibitor combinations (nivolumab + ipilimumab, pembrolizumab + axitinib, nivolumab + cabozantinib) and VEGFR TKI monotherapy. While these combinations have achieved durable responses in a subset of patients, the majority eventually develop resistance, and non-clear cell RCC subtypes (papillary, chromophobe, collecting duct) remain significantly undertreated.
