About Fibromyalgia

Fibromyalgia is a chronic pain disorder characterized by widespread musculoskeletal pain, fatigue, sleep disturbances, and cognitive difficulties ("fibro fog"). It affects approximately 4 million Americans (2% of the adult population) and is more prevalent in women. Fibromyalgia is a disorder of central pain processing (central sensitization) rather than peripheral inflammation — tissues are structurally normal, but the nervous system amplifies pain signals. FDA-approved treatments include duloxetine, milnacipran, and pregabalin, but fewer than 50% of patients achieve meaningful symptom improvement.

What Types of Fibromyalgia Clinical Trials Exist?

Fibromyalgia trials test new pharmacological targets (sodium channel modulators, NK1 receptor antagonists, low-dose naltrexone, sigma-1 receptor antagonists), non-pharmacological interventions (CBT-based digital platforms, mindfulness, graded exercise programs), transcranial magnetic stimulation and other neuromodulation approaches, and combination mind-body interventions. The Revised Fibromyalgia Impact Questionnaire (FIQR) and Numeric Pain Rating Scale are standard eligibility and endpoint measures. ACR 2010/2016 diagnostic criteria are used to confirm the diagnosis.

Frequently Asked Questions

What clinical trials are available for fibromyalgia?
Recruiting fibromyalgia trials include novel pharmacological agents (low-dose naltrexone, sigma-1 receptor antagonists, sodium channel modulators), cognitive behavioral therapy digital programs, transcranial magnetic stimulation (TMS), mindfulness-based stress reduction, graded exercise and physical therapy protocols, and combination mind-body intervention studies.
What criteria are used to diagnose fibromyalgia for clinical trials?
The ACR 2010/2016 criteria (Widespread Pain Index and Symptom Severity Scale) are standard for trial enrollment — no tender point examination is required. Most trials require a minimum pain severity score (NRS ≥5/10 or FIQR total ≥39) to confirm meaningful disease burden. A history of symptom duration ≥3 months is standard.
Can I join a fibromyalgia trial if I am on current medications?
Depends on the trial. Some trials allow stable background medications (antidepressants, pregabalin) and test add-on therapies. Others require washout of existing fibromyalgia treatments. Trials testing new central sensitization targets may require medication-free periods to avoid confounding. Document your current medications and doses carefully.
