About Chronic Kidney Disease

Chronic kidney disease (CKD) affects approximately 37 million Americans (15% of adults) and is defined as abnormalities of kidney structure or function present for more than 3 months. It is staged by eGFR (estimated glomerular filtration rate) from G1 (eGFR ≥90) through G5 (eGFR <15, kidney failure). Diabetic nephropathy and hypertensive nephropathy are the leading causes. CKD is a major cardiovascular risk factor and leads to end-stage renal disease (ESRD) requiring dialysis or transplantation in a subset of patients. SGLT2 inhibitors (dapagliflozin, empagliflozin) have recently demonstrated kidney-protective effects across CKD causes.

What Types of Chronic Kidney Disease Clinical Trials Exist?

CKD trials span multiple etiologies and stages. IgA nephropathy (IgAN) — the most common primary glomerulonephritis — has a newly approved targeted therapy (sparsentan, budesonide) and multiple active trials targeting the complement pathway, APRIL/BAFF, and endothelin. FSGS (focal segmental glomerulosclerosis) trials test sparsentan and novel agents. Diabetic nephropathy trials extend SGLT2 inhibitor and non-steroidal MRA (finerenone) data.
